BT1 3HHEnsuring the request states your reason for changing names (eg Marriage, Divorce, Deed Poll), signed with your old signature and signed with your new signature. Please also provide a certified copy of the formal document that clarifies your name change (eg Marriage Certificate, updated Identification, proof of address, Legal Documents).
